Job summary

Lutheran Homes Group is seeking a Transitional Care Program (TCP) Coordinator to deliver assessment, care planning and case management in the community/homebased TCP program. You will work closely with consumers, families, allied health professionals, nursing teams and external providers to support independent living and well-being.

The role requires registration with AHPRA, a commitment to high-quality care, and the ability to coordinate discharge planning and service delivery across settings.

Qualifications

  • Registration with AHPRA and current practising certificate.
  • Minimum 3 years post-registration clinical experience.
  • Experience in quality improvement activities.
  • Strong communication, stakeholder engagement and problem-solving.
  • Ability to work independently within a multidisciplinary team.
  • Consumer-focused approach to high-quality care.
  • Current driver's licence and willingness to travel.

About the Role

As our Transitional Care Program (TCP) Coordinator, you will be involved in providing assessment, support, care planning, and case management to consumers and their support persons within the community/homebased TCP program. Working closely with consumers, families, allied health professionals, nursing teams and external service providers, to deliver high-quality case management aimed at achieving optimal independence and well-being for consumers.

Key Responsibilities
  • Coordinate Transitional Care Program services, ensuring high-quality, person-centred care.
  • Conduct assessments, care planning and case management aligned with TCP guidelines and consumer goals.
  • Support seamless transitions between hospital, residential and community settings through effective discharge planning and service coordination.
  • Collaborate with consumers, families and healthcare stakeholders to maximise independence and restorative outcomes.
  • Maintain accurate records and contribute to service quality and continuous improvement initiatives
What You Will Need
  • Registration, or eligibility for registration, as a Health Practitioner with AHPRA and a current practising certificate.
  • Minimum 3 years' post-registration clinical experience and demonstrated competence in relevant practice areas.
  • Experience in quality improvement activities and applying evidence-based practice.
  • Strong communication, stakeholder engagement, organisational and problem-solving skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • A consumer-focused approach with a commitment to high-quality care outcomes.
  • Current Driver's Licence and willingness to travel within the community.

Candidates will be required to undergo a pre-employment medical assessment and must hold a current NDIS Worker Clearance. In line with current health guidelines for the aged care sector, candidates are strongly encouraged to be up to date with all recommended vaccinations.
