Job summary

Lutheran Homes Group is seeking a Transitional Care Program (TCP) Coordinator to provide assessment, care planning, and case management for community/home-based TCP services. You will work with consumers, families, allied health professionals, and nursing teams to support safe, home-based transitions and independence.

The role requires registration with AHPRA, 3+ years post-registration experience, and a consumer-focused approach to high-quality care outcomes, with travel within the community

Qualifications

• Registration with AHPRA and current practising certificate.
• Minimum 3 years' post-registration clinical experience.
• Experience in quality improvement and applying evidence-based practice.
• Strong communication, stakeholder engagement, organisational and problem-solving skills.
• Ability to work independently and collaboratively within a multidisciplinary team.

Responsibilities

• Coordinate Transitional Care Program services with high-quality, person-centred care.
• Conduct assessments, care planning and case management aligned with TCP guidelines.
• Support transitions between hospital, residential and community settings through discharge planning and service coordination.
• Collaborate with consumers, families and healthcare stakeholders to maximise independence.
• Maintain accurate records and contribute to service quality and continuous improvement.
