Job summary

Maxlife Care, Seven Hills, Sydney, is seeking a Team Leader for Disability Support to guide a team of DSWs delivering personalised, high-quality care. You will ensure adherence to care plans and collaborate with the Area Manager to drive success.

You will supervise staff, manage rosters, and provide hands-on support when required, including personal care and daily living activities, while maintaining compliance with NDIS standards and organisational policies.

• Ability to manage competing priorities and be proactive.

Responsibilities

• Leading, supervising and developing your team of Disability Support Workers.
• Escalating concerns, incidents and complex matters to your Area Manager promptly and accurately.
• Supporting the Roster Coordinator to build rosters four to six weeks in advance.
• Ensuring all client care plans, including Behaviour Support Plans, are followed and reviewed as needed.
• Building strong relationships with clients, families, Support Coordinators and allied health professionals.
• Maintaining compliance with NDIS Practice Standards, legislation and policies, including mandatory reporting.
• Providing hands-on support to clients, including personal care, meal prep, medication administration and daily living activities.

Maxlife Care is a doctor-led NDIS provider delivering clinically robust, person-centred care to clients with complex and high-acuity needs. We are the only Supported Independent Living provider in NSW with an in-house team of GPs and nurses available 24/7.
We are growing rapidly across NSW and are looking for experienced Team Leaders to join our SIL house in Seven Hills, Sydney.

About the Role
As a Team Leader, you will lead a team of Disability Support Workers, maintain the standards of care Maxlife Care is known for, and work closely with your Area Manager who will provide guidance, development and oversight to set you and your team up for success.
Your key responsibilities will include:

• Leading, supervising and developing your team of Disability Support Workers.
• Escalating concerns, incidents and complex matters to your Area Manager promptly and accurately.
• Supporting the Roster Coordinator to build and manage rosters four to six weeks in advance.
• Ensuring all client care plans, including Behaviour Support Plans, are followed consistently and reviewed when client needs change.
• Building strong relationships with clients, families, Support Coordinators and allied health professionals.
• Maintaining compliance with NDIS Practice Standards, relevant legislation and Maxlife Care policies, including mandatory reporting.
• Providing hands-on support to clients as required, including personal care, meal preparation, medication administration and daily living activities.

About You

• Minimum three years experience supervising or leading a team of Disability Support Workers in a SIL or group home setting.
• Experience supporting clients with complex needs, including psychosocial disability, physical disability, intellectual disability, autism spectrum disorder and complex behaviours.
• Certificate III in Individual Support (Disability) or equivalent. Certificate IV or Diploma is an advantage.
• Strong communication skills and the ability to build rapport with clients, families and team members.
• Proactive, solutions-focused and able to manage competing priorities.
• Current NSW Driver's Licence and access to your own vehicle is desirable.
