Job summary

Anglicare Sydney is seeking a Tailored Support Coordinator to assess resident needs, develop practical support plans and connect people with Anglicare services and community providers. You will coordinate services, monitor progress and maintain confidential case records to help improve health, wellbeing and independence.

We value compassionate, organised practitioners with strong communication and advocacy skills, and experience delivering social services.

Qualifications

  • Qualification in Social Work, Community Services, Allied Health, Aged Care, Community Housing or related field.
  • 12 months post-qualifying experience delivering community-based social services.
  • Experience coordinating services for people with complex needs and maintaining accurate case documentation.
  • Strong communication, advocacy and stakeholder relationship skills.
  • Current driver licence and ability to meet probity/compliance checks.

Responsibilities

  • Assess resident needs and develop tailored support plans with clear, achievable outcomes.
  • Connect residents with health, legal, financial, advocacy, community and Anglicare services.
  • Coordinate case discussions and build productive relationships with residents, families and service providers.
  • Monitor support plans, resident progress, service quality and budgets; respond to changing circumstances.
  • Maintain accurate and confidential case records and contribute to wellbeing reporting.

Your opportunity

Reporting to the Senior Tailored Support Coordinator, you’ll assess resident needs and work collaboratively with each person to develop practical, achievable support plans. You’ll connect residents with Anglicare services, community providers and specialist supports, while coordinating services and monitoring progress to help achieve stronger social, health, recovery and wellbeing outcomes. With integrity and resilience, you will: Complete resident needs assessments and develop tailored support plans with clear, achievable outcomes. Connect residents with health, legal, financial, advocacy, community and Anglicare services. Coordinate case discussions and build productive relationships with residents, families and service providers. Monitor support plans, resident progress, service quality and agreed budgets, responding when needs or circumstances change. Maintain accurate and confidential case records and contribute to wellbeing, participation and tenant satisfaction reporting.

What you’ll bring

You’re a compassionate and organised practitioner who knows how to listen, assess individual needs and coordinate the right supports to help people achieve meaningful outcomes. You’ll bring strong case coordination and relationship‑building skills, together with an understanding of how housing, culture, crisis and complex needs can shape a person’s experience. You also share our organisational values of integrity, justice, compassion and excellence, embedding them in everything you do.

What’s required
  • Qualification in Social Work, Community Services, Allied Health, Aged Care, Community Housing or a related field, and/or relevant experience.
  • At least 12 months post‑qualifying experience delivering community‑based social services, including assessment and care coordination.
  • Experience coordinating services for people with complex needs and maintaining accurate case documentation.
  • Strong communication, advocacy and stakeholder relationship skills.
  • Current driver licence and ability to meet the role’s required probity and compliance checks.
What’s preferred
  • Experience supporting residents within social or affordable housing, aged care or community services.
  • Understanding of evidence‑based, culturally safe and trauma‑informed practice.
  • Knowledge of community services, referral pathways and multidisciplinary support networks.
