• Ability to build rapport quickly with participants and families.

• Experience in mental health or disability outreach and supporting people with complex needs.

Responsibilities

• Provide outreach support and telehealth alongside a Dandenong South team.
• Coordinate with care teams and link participants to services.
• Develop relationships with participants, families and community partners.
• Manage referrals, complete risk assessments and maintain case notes and reports.
• Navigate multiple systems and platforms used in care coordination.
