ermha365 invites applications for a Support Facilitator within its EIPSR program in Geelong. The role operates on a hybrid model, including home-based work, Geelong office duties, and outreach across the Greater Geelong region and Bellarine Peninsula.
The position is a maximum term part-time (0.8 FTE) ending 30/06/2027, requiring compliance with NDIS Work Screening, WWCC, work rights, and a current Victorian driver’s licence.
At ermha365, we are part of Meridia and we work with clients with some of the most complex mental health and disability support needs in Victoria. We’re a lifeline for people who often feel like a “square peg in a round hole” - those who experience stigma and discrimination, and are ostracised or excluded from the simple things that most of us take for granted. In many cases we have become the provider of last resort, although we see ourselves as the provider of first choice. We believe in the potential of everyone. We never give up, and we never give in.
ermha365 provides the Early Intervention Psychosocial Support Response (EIPSR) program in partnership with Barwon Health. EIPSR provides specialist psychosocial support to help eligible participants to build their capacity to better manage their mental health, develop practical life skills for independent living and social connectedness, and if eligible, support with exploring pathways to support needs.
Working under the leadership of the Practice Leader, The Support Facilitator’s goal is to empower rather than to do; to promote independence, development and confidence in the participant. Ideally, we are looking for a candidate with Lived Living Experience or non Lived Living Experience and have the ability to (but not limited to):
