Claro Disability Services is seeking a compassionate care professional to support clients with complex needs in a community-based setting. You will work closely with families and medical teams to deliver personalized care plans that enhance independence and wellbeing.

Responsabilités

• Person-centered planning: chat with the client, family, and advocates to understand goals and routines.
• Complex physical care: assist with mobility, transfers, positioning, daily tasks and devices per plan.
• Medical supports: monitor changes, follow clinical plans, liaise with the care team as needed.
• Social time and participation: provide meaningful activities aligned with client interests.
• Autonomy and advocacy: support decision-making, respect consent, and boost independence.
• Cultural humility: respect backgrounds, beliefs, and ways of being.

Shift Times

Tuesday and Thursday 630-830am.

What you’ll do (*tailored to each client as required*)

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being

• Reliable and professional, with good notetaking and teamwork.

• Optional but nice: Manual handling, epilepsy management, or disability-support training.
