Job summary

Claro Disability Services – Pillar Valley NSW seeks a compassionate Support Worker to assist clients in the Clarence Valley with mobility, daily tasks, and social participation. You will work closely with families, medical professionals, and other workers to deliver person-centered care that aligns with goals and routines.

Responsibilities

• Support decision-making and promote autonomy while respecting consent.

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
