Job summary

Claro Disability Services in Newcastle area is seeking compassionate support workers to assist people with complex needs to live independently. You will tailor supports to each client's goals and routines, collaborating with families, medical professionals, and other care workers to ensure continuity of care.

Responsibilities

• Deliver person-centered care across mobility, daily tasks, and routines.

• Collaborate with families and medical professionals to coordinate care.

Shifts available

• Fortnightly Mon 0730-1400
• Fortnightly Fri 0730-1400

What you’ll do (*tailored to each client as required*)

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
