Job summary

Claro Disability Services on the Gold Coast is seeking compassionate support workers to assist clients with complex care. You will partner with families and healthcare teams to implement person-centered plans, support daily living, mobility, and independence.

Responsibilities

• Person-centered planning: Chat with the client and family/advocates to understand goals and routines.
• Complex physical care: Mobility, transfers, positioning, daily tasks, and devices as outlined in the plan.
• Medical supports: Monitor changes, follow clinical plans, and liaise with the care team.
• Social time and participation: Provide meaningful activities aligned with client interests.
• Autonomy and advocacy: Support decision-making, respect consent, and boost independence.
• Cultural humility: Respect backgrounds, beliefs, and ways of being.

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
