Job description

We are looking for an enthusiastic and experienced individual to work as a member of a small team to provide support to a man in his thirties. You will welcome the opportunity to focus your skills and experience to make a significant difference in the life of this man who is embracing living independently in his own home and community.

He takes pride in his home, his independence and in his achievements. He enjoys the company of family and close friends, having a laugh and volunteering his time in the community. He is known for his love of the outdoors; picnics, bush walking, camping and sailing. He has limited verbal skills, however he is intelligent, perceptive and a keen observer of life. He possesses an inner strength and resilience, calmly facing the challenges in his life as he strives for growth, personal development and fulfilment.

He is surrounded by a strong informal support system and social framework that includes his family and informal networks, who play a central role in his life, providing encouragement, guidance and oversight. As such, his life is real, dynamic and sometimes messy and each of his team members will need to embrace this in their role. He has a dedicated small team of support workers who assist him in daily tasks and help him navigate independent living.

We are seeking a male or female support worker who:

Will collaborate closely with the young man, his family, and informal supports to provide exceptional, person-centred and developmental support. Your role will focus on doing with, not for, empowering him to actively participate in his daily life, make choices, decisions and promote independence in all activities.

Your duties will include:

  • Supporting the person with their communication, implementing their multiple, complex, communication strategies.
  • Supporting the person’s personal dignity and well being in all activities and at all times.
  • Assisting the person with daily activities, including personal care, meal preparation, household chores, and transportation.
  • Supporting social and community participation, including attending events, outings, and appointments.
  • Supporting the person to thrive in his home by maintaining his routines and rhythm, ensuring his environment remains safe, welcoming, and conducive to self-expression, while also creating opportunities for him to invite important others in as he chooses.
  • Supporting him to attain valued roles and maintain and develop relationships.
  • Developing and maintaining positive relationships with the person, his family, and informal supports.
  • Following individual support plans and providing feedback to the Supervising Team Leader and other professionals as required.
  • Maintaining clear and accurate documentation such as shift notes, incident reports and medication records.

About You

You are a compassionate and proactive individual with a strong commitment to supporting people with disabilities. You demonstrate respect for the rights, dignity, and independence of others, and you are eager to make a meaningful difference in their lives.

You will:

  • Have experience working with individuals with disabilities or in another industry where you are able to demonstrate a clear ability to follow clear routines and processes.
  • Be passionate about person-centred support and empowering individuals to achieve their hopes and goals.
  • Embrace a growth mindset, doing ‘with not for’ the person, recognising that our work is an ongoing journey and that there is always room for growth, development, and improvement.
  • Value and commit to supporting the natural authority of the family in decision making with/for this man.
  • Have a ‘can do’ attitude and pride yourself on being calm and kind.
  • Be flexible, reliable, and resourceful with the ability to adapt to changing circumstances.
  • Use initiative, common sense, innovation in providing support while also being open to taking direction, and feedback from the person, their family and team leader.
  • Have the ability to work collaboratively and positively with other support workers and stakeholders.
  • Have strong communication and interpersonal skills and a natural ability to build rapport.
  • Have excellent time management, attention to detail and organisational skills.
  • Be comfortable using technology for record-keeping, following written documentation, routines and communication.
  • Previous experience working in disability support and have a high attention to detail.
  • Understanding of person-centred, strengths-based, and rights-focused approaches.
  • Strong communication and interpersonal skills.
  • Ability to work as part of a team and independently.
  • NDIS Worker Screening clearance.
  • Current First Aid and CPR certificates.
  • A car, valid driver's license, and vehicle insurance.
