Job summary

Lutheran Disability Services is seeking an experienced Support Worker to help a client living at home and engaging with the community. You will provide respectful, person-centred supports, assist with daily living, transport, personal care and recreational activities.

The role requires a minimum of two years experience, knowledge of autism, strong communication, and a flexible approach. You will work with the client's family and other professionals, including availability for day and overnight

Qualifications

• Minimum 2 years of experience in disability support.
• Experience in supporting a person living with Autism is preferred.
• Compassionate, client-centred and respectful approach required.

Responsibilities

• Support the client with daily living activities, including routine setting, personal care, meal preparation, mobility support and medication management.
• Ensure the client participates in decision-making regarding their care and day-to-day supports.
• Assist the client to access community resources, services, day options and recreational activities.
• Encourage the client to build confidence, develop skills and work towards personal goals.
• Provide emotional support in a respectful and empowering way.
• Collaborate with the client’s family and other professionals to support individual needs.
• Document client progress and report changes in behaviour, wellbeing or health to the team.
• Maintain a safe environment and follow health and safety procedures.

About this role

We are looking for an experienced (minimum 2 years) Support Worker, to assist a client living in their own home and accessing the local community. You will have relevant knowledge in supporting a person living with Autism. Your role will include providing respectful, caring, person-centred supports and assisting the client with everyday activities including; recreational activities, transport, personal care, building friendships and working towards achieving personal goals. Your supports will encourage the persons independence, choice and inclusion in all areas of their life.

What you’ll be doing

• Supporting the client with daily living activities, including routine setting, personal care, meal preparation, mobility support and medication management.
• Ensuring the client is actively engaged in all decision making relating to their care and day to day supports
• Assisting the client to access community resources, services, day options and recreational activities.
• Encouraging the client to build their confidence, develop skills and work towards their personal goals.
• Providing emotional support in a respectful and empowering way.
• Working collaboratively with the client’s families and other professionals to support individual needs.
• Documenting client progress and reporting changes in behaviour, wellbeing or health to the appropriate team members.
• Maintaining a safe environment by following health and safety procedures, reporting hazards and supporting cleanliness.
• Participating in training (including pool safety) to stay up to date with best practice in disability support.
• You will be available for both day and overnight shifts and be able to support others in your team at short notice (emergency)
