Job summary

DFP Recruitment is seeking experienced professionals to deliver high-quality, person-centred support needs assessments for people living with disability across the Northern Territory. You will gather information, interpret evidence, and prepare clear assessment reports to support informed decisions.

You will engage respectfully with participants, families and support networks using trauma-informed and culturally-safe approaches to ensure people feel heard and supported.

Qualifications

• Experience conducting support needs assessments or similar person-centred assessments.
• Strong analytical, report writing and decision-making capability.
• Commitment to inclusive, accessible and culturally-safe practice.
• Trauma-informed and strengths-based approaches to participant engagement.
• Ability to manage competing priorities in a fast-paced environment.
• High levels of professionalism, integrity and ethical practice.
• Experience working within disability, health, allied health or community services environments.

Responsibilities

• Conduct support needs assessments through guided conversations and information gathering.
• Analyse and synthesise participant information and external evidence to support evidence-based outcomes.
• Prepare clear, accurate and defensible written assessment reports.
• Apply policy, legislation and procedures using sound judgement and ethical practice.
• Communicate assessment processes and outcomes clearly across virtual and face-to-face settings.
• Escalate complex or high-risk matters appropriately and contribute to continuous service improvement.
• Collaborate across teams to support coordinated and inclusive participant outcomes.
• Maintain accurate records using digital systems while ensuring privacy and confidentiality.
