Job summary

DFP Recruitment is seeking experienced professionals to deliver high-quality, person-centred support needs assessments for people living with disability. In this role, you will gather and analyse information, interpret evidence, and prepare clear, accurate assessment reports to support informed decision-making.

Positions are hybrid WFH up to 3 days per week with base office locations across Queensland; Australian Citizens; assignments through until 31 December 2026 with view to extension.

Qualifications

• Must hold a recognised tertiary qualification in allied health or a relevant health discipline.
• Evidence of qualifications must be included in the CV.
• Experience or accreditation in the I-CAN Support Needs Assessment Tool is highly regarded but not essential.

Responsibilities

• Conduct support needs assessments through guided conversations and information gathering.
• Analyse and synthesise participant information and external evidence to support evidence-based outcomes.
• Prepare clear, accurate and defensible written assessment reports.
• Apply policy, legislation and procedures using sound judgement and ethical practice.
• Communicate assessment processes and outcomes clearly across virtual and face-to-face settings.
• Escalate complex or high-risk matters appropriately and contribute to continuous service improvement.
• Collaborate across teams to support coordinated and inclusive participant outcomes.
• Maintain accurate records using digital systems while ensuring privacy and confidentiality.
