Cobden Health is seeking a dedicated Clinical Care Partner (Registered Nurse) Part-time/Casual to join our growing Support at Home team. This role provides the opportunity to support older people to remain independent, safe and well in their own homes while delivering high quality, person-centred care. The position is responsible for clinical assessments, care planning, care coordination and ongoing case management for clients receiving Support at Home services.
