Job summary

Home At Heart is seeking an experienced Support at Home Care Partner to join the team. Based primarily in Griffith, with fortnightly travel to Griffith from Wagga Wagga and occasional overnight stays when based in Wagga Wagga.

You will coordinate services, monitor care delivery, and ensure participants receive high-quality, person-centred support. The role focuses on helping older Australians stay independent and safe at home, collaborating with families, support workers and health professionals

Qualifications

• Experience in care coordination, case management or similar aged care or disability role.
• Strong understanding of home care services and aged care sector.
• Excellent communication and relationship-building skills.
• Ability to manage competing priorities and work independently.
• Strong computer and administration skills.
• Current driver's licence.
• Willingness to travel and stay overnight when required.
• Qualification in Nursing, Allied Health, Community Services, Ageing Support or related field.

Responsibilities

• Coordinate and manage a portfolio of Support at Home participants.
• Undertake participant assessments and reviews.
• Develop, implement and monitor individual support plans.
• Liaise with participants, families, support staff and external service providers.
• Ensure services are delivered in accordance with participant goals, funding requirements and organisational policies.
• Monitor participant wellbeing and identify changing care needs.Maintain accurate participant records and documentation.
• Support compliance with aged care quality standards and relevant legislation.

About the Role

An exciting opportunity is available for an experienced Support at Home Care Partner to join our team. This role is ideally based in Griffith, however applicants located in Wagga Wagga are encouraged to apply, noting that regular fortnightly travel to Griffith will be required. Occasional overnight travel will be required as part of this role if based in Wagga Wagga.

This role is ideal for someone who is passionate about helping older Australians remain independent and living safely in their own homes. Working closely with participants, families, support workers and health professionals, you will coordinate services, monitor care delivery, and ensure participants receive high-quality, person-centred support.
