Job summary

Volunteers of America Delaware Valley is seeking a skilled case manager to conduct risk/needs assessments and coordinate treatment referrals. You will develop client-centered service plans, monitor progress, and ensure engagement with services while maintaining confidentiality.

The role requires a Bachelor's degree in Human Services or related field, plus experience with addictive behaviors, strong assessment skills, and valid driver's license.

Qualifications

• Bachelor's degree in Human Services or related field.
• Five years of experience with individuals with addictive behaviors.
• Skilled in assessment and evaluation; basic CBT concepts preferred.
• Effective written and verbal communication; interact respectfully with clients.
• Valid driver's license.

Responsibilities

• Assess risk/needs to determine counseling and level of care.
• Develop client-centered service plans with client input.
• Monitor progress and document per funding/source standards.
• Schedule and facilitate individual sessions and groups.
• Coordinate referrals and link clients to needed services.
• Complete required reports and participate in treatment team meetings.
