Job summary

Volunteers of America Delaware Valley is hiring an Outreach Worker to serve unhoused individuals in Camden. You will provide information, assess emergency needs, and assist with intake and discharge planning, working as part of a team to promote client self-determination.

You will facilitate group sessions, coordinate transportation, and connect clients with services while maintaining confidentiality and respectful communication with staff and community partners.

Qualifications

  • Minimum two years' work experience in a relevant field.
  • Direct experience with unhoused populations and individuals who suffer from substance abuse and mental health issues.
  • Must possess practical written and verbal communication skills.
  • Must pass a criminal background check.
  • Bilingual is a plus.

Responsibilities

  • Provide information to homeless individuals and community members.

Job description

Responsibilities include providing information to homeless vulnerable populations in unsafe conditions; assessing the emergency needs for each individual requesting service; providing group facilitation, and working as part of the team for effective delivery of services. Provide highly individualized services in the community and promote client self-determination and decision-making. Provide essential expertise and consultation to the team to promote a culture in which each client's point of view and preferences are recognized, understood, respected, and integrated into treatment, rehabilitation, and community self-help activities.

  • Participate in regular street outreach activities to encourage homeless individuals to enter into a program and participate in law enforcement outreach initiatives.
  • Disseminate information regarding services provided by the agency to the community.
  • Responsible for the coordination of transportation for individuals requesting services.
  • Coordinate comprehensive intake assessment procedures for all residents coming in as a result of outreach activities.
  • Communicates special needs and represents client's interests to program staff.
  • Network with other community agencies to develop resources for clients.
  • Complete Service Plans on all assigned clients based on approved agency assessment tools with client input.
  • Monitor and document client progress according to funding source standards and/or operational
  • Develop appropriate discharge plans for all assigned clients as per contractual requirements and policy and procedures.
  • Complete necessary reports and case-related paperwork in a timely fashion.
  • Facilitate regular group sessions for clients as directed.
  • Provide practical assistance for clients when they first enter the facility by mentoring, advocacy, coordination, side-by-side individualized support, problem-solving, and direct assistance to help clients obtain the necessities of daily living including utilizing nutritional, financial, physical, mental, and emotional resources.
  • Assist in maintaining communication between all staff members and clients.
  • Maintain client and staff confidentiality in compliance with administrative policies and procedures.
  • Attend and participate in regularly scheduled staff meetings, supervisory conferences, and training sessions and meetings as required.
  • Interact verbally and non-verbally with all clients, staff, and community members with respect and courtesy.
Qualifications
Education
Required

High School or better.

Experience
Required

High School Diploma or equivalent with a minimum of two years' work experience in a relevant field. Direct experience in working with unhoused/homeless population and individuals who suffer from substance abuse and mental health issues. Must possess practical written and verbal communication skills and an ability to interact appropriately with clientele. Must be able to pass a criminal background check. Bilingual is a plus.
