Job summary

SA Health in Barossa Region seeks a Nurse Consultant to lead the Aged Care Transition initiative, driving improvements in residential aged care referral and transfer processes across the health system.

You will collaborate with clinicians, executives and statewide stakeholders to shape service delivery models and reform, with a focus on quality, safety, and continuity of care for older people transitioning to residential aged care.

Qualifications

• Registered Nurse with extensive aged care experience.
• Experience in care transitions, project implementation or service improvement initiatives.
• Ability to lead and influence change through partnerships and collaboration.

Responsibilities

• Lead the Residential Aged Care Documentation Project implementations and evaluations.
• Drive improvements in aged care transition processes with evidence-based documentation.
• Collaborate with clinicians, executives, government agencies and aged care providers to achieve outcomes.
• Monitor progress using data, research and performance metrics; provide recommendations.
• Provide clinical leadership, coaching and guidance to support organizational change and workforce capability.
