About us

HM Prison Barwon is a maximum-security prison located 6km north-west of Lara, 20km from the centre of Geelong and about 70km from Melbourne and is part of the Barwon Southwest Regional structure.

It is Victoria's major maximum-security prison for sentenced male prisoners. The prison officially opened in January 1990 and currently houses up to 416 men in custody, including high security, long term management, protection (including specialist protection), general mainstream (maximum security) and medium security prisoners.

Corrections Victoria is responsible for the implementation of policies, programs and services that ensure the safe and secure containment of prisoners, and that seek to rehabilitate offenders by addressing the underlying causes of offending behaviour.

What you will do

As Specialised Lead Assessment Transition Coordinator, you will coordinate transition services for people in custody. You will help people with complex needs prepare for release and connect with support services.

You will:

• coordinate the specialised Corrections Victoria Reintegration Pathway for people with complex needs.
• complete risk and reintegration assessments and support parole applications.
• work with multidisciplinary teams to plan release and transition services.
• coordinate referrals with prison staff and external service providers.
• maintain accurate records, data and reports on services and outcomes.

About you

No mandatory qualification is listed for this role. Experience delivering assessments, programs or services in corrections is highly desirable.

You should also have:

• detailed knowledge of the Corrections Victoria Reintegration Pathway and complex transition needs.
• strong assessment skills and experience using risk and other assessment tools.
• clear advisory skills to guide colleagues through training, coaching and knowledge sharing.
• correctional experience and the ability to work well with different teams.
