Job summary

Amplar Health, part of the Medibank Group, seeks a full-time Social Worker to deliver comprehensive care for patients with varying complexity in Adelaide's CBD.

Collaborate with a multidisciplinary team, assess and triage, coordinate services, and participate in clinical reviews. Candidates should hold a Social Work degree with AASW membership, and meet child safety, police clearance, and vaccination requirements.

Qualifications

• Degree in Social Work with membership with the Australian Association of Social Workers (AASW).
• Experience as a Social Worker in sub-acute or similar healthcare setting.
• Strong communication, advocacy, and problem-solving skills.
• Excellent communication and interpersonal skills with adaptability to evolving service needs.
• Proficiency in Microsoft Office and documentation systems.
• South Australian Working with Children Check and National Police Clearance or willing to obtain.
• Willing to provide proof of up-to-date vaccinations if successful.

Responsibilities

• Collaborate with a multidisciplinary team to develop and implement integrated service models and clinical protocols.
• Facilitate group sessions and contribute to health promotion activities as required.
• Provide effective assessment and triage, timely referrals, accurate information, coordinated care and prompt follow up.
• Advocate for patient's needs and coordinate access to services
• Participate actively in clinical review meetings and continuous professional development.

We are looking for a full time Social Worker to deliver comprehensive care for to patients with varying complexity on a full-time basis centrally in Adelaide's CBD.

Working in analternate dynamic environment, you will utilise innovative technology and be rewarded in a role where your expertise will contribute to shaping exceptional health outcome for sub-acute patients.Here’s a glimpse of what you will be doing:

• Collaborate with a multidisciplinary team to develop and implement integrated service models and clinical protocols.

About you

You are passionate about the provision of innovative models of care and excited by this dynamic way of working. Additionally, you have:

• Degree in Social Work, with membership with the Australian Association of Social Workers (AASW).
• Demonstrated experience as a Social Worker in a sub-acute or similar healthcare setting.
• Strong communication, advocacy, and problem-solving skills.
• Excellent communication and interpersonal skills, with the ability to adapt to evolving service need.
• Proficiency in using Microsoft Office suite and other relevant documentation systems.
• Hold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain).
• If successful, willing and able to provide proof of up-to-date vaccinations.
