Job summary

ACH Group is seeking a Social Links Community Connector to support customers in identifying preferences and connecting with social experiences within ACH Group and the wider community. You will work closely with the Social Community Team to promote and grow Social Link experiences, matching customers to suitable activities using a person-directed model.

Qualifications

  • Team player with a strong customer service approach.
  • Excellent interpersonal and communication skills.
  • Holds a current driver’s licence and access to a reliable vehicle.
  • Tertiary level education in health or community fields or actively pursuing a related certificate.

Responsibilities

  • Identify preferences and connect customers with social experiences at ACH Group or in the community.
  • Collaborate with Social Community Team to promote Social Link experiences.
  • Match customers to relevant social activities using a person-centred approach.
  • Support new customers to join groups and overcome participation barriers.
  • Provide time-limited planning and coaching to high-risk customers and discuss risk plans with Care Manager.

About the Role:

ACH Group have a 0.8 FTE part time – fixed term opportunity until 5 March 2027, within the Home Care Community program based at the North office.

What you’ll do:
  • Support customers to identify preferences and connect with social experiences at ACH Group and/or in the broader community
  • work with the Social Community Team to promote and grow the Social Link experiences
  • match customers to relevant social experiences including ACH social experiences and wider social opportunities using a person directed model
  • follow-up customer referrals and support new customers to feel comfortable joining groups or connecting with individual support – assist customers to overcome barriers to participation
  • provide time-limited individual planning, support & coaching to complex, high risk customers to meet social goals and discuss risk management plans with Care Manager. (e.g. memory loss or mobility)
  • Work internally with teams to support customer’s social needs and queries
  • Participate in group co-design and evaluation utilising insights into customers’ preferences
  • use ‘soft’ sales skills to promote products and build positive relationships with internal and external business partners and customers
  • Positively contribute to a team environment
  • knowledge of Ecliptix &/or willing to learn

About you:

  • Hold a current driver’s licence, that complies with Australian licencing laws and regulations, with access to a registered reliable vehicle
  • Applicants that also hold Tertiary level education in health, community, or other relevant area or Certificate IV in Community Services/ Aged Care/ Service Coordination or working towards
