Job summary

Calvary Riverina Hospital in Wagga Wagga, NSW is seeking a compassionate Social Worker to provide psychosocial assessment, support and advocacy across the hospital, with a focus on rehabilitation, palliative care and complex health journeys.

This permanent, part-time role involves discharge planning, care coordination and working with a multidisciplinary team to support patients and families through illness, recovery and end-of-life care. Monday to Friday, business hours.

Qualifications

• Degree in Social Work and AASW eligibility.
• Current National Police Check or willingness to obtain.
• Experience in hospital, rehabilitation or palliative care settings.
• Strong communication and multidisciplinary teamwork skills.
• Ability to manage complex caseloads and competing priorities.

About the role

Calvary Riverina Hospital is seeking a compassionate and experienced Social Worker to provide psychosocial assessment, support and advocacy across the hospital, with a key focus on our Palliative Care and Rehabilitation Units.

This permanent position offers the opportunity to work as part of a multidisciplinary team, supporting patients and their families through illness, recovery, adjustment and end-of-life care. You will play an integral role in discharge planning, care coordination and facilitating access to community supports and services.

This is a part-time position based in Wagga Wagga, NSW, working business hours Monday to Friday.

Your main duties will include:

• Provide psychosocial assessment, counselling and support to patients and families
• Discharge planning and coordination of referrals to community and support services
• Collaborate with multidisciplinary teams to optimise patient care and outcomes
• Participation in weekly MDT meetings in the rehabilitation unit

About you
