Job summary

Calvary Riverina Private Hospital is seeking a compassionate Social Worker to provide psychosocial assessment, support and advocacy across the hospital, focusing on the Palliative Care and Rehabilitation Units.

You will participate in discharge planning, care coordination and access to community supports, working within a multidisciplinary team to improve patient outcomes and family support.

Qualifications

• Degree in Social Work and AASW eligibility.
• Current National Police Check or willingness to obtain.
• Experience in hospital, rehabilitation or palliative care settings.
• Strong communication and multidisciplinary teamwork skills.
• Ability to manage complex caseloads and competing priorities.

Responsibilities

• Provide psychosocial assessment, counselling and support to patients and families.

About the role

Calvary Riverina Hospital is seeking a compassionate and experienced Social Worker to provide psychosocial assessment, support and advocacy across the hospital, with a key focus on our Palliative Care and Rehabilitation Units.

This permanent position offers the opportunity to work as part of a multidisciplinary team, supporting patients and their families through illness, recovery, adjustment and end-of-life care. You will play an integral role in discharge planning, care coordination and facilitating access to community supports and services.

This is a part-time position based in Wagga Wagga, NSW, working business hours Monday to Friday.

Your main duties will include:

• Provide psychosocial assessment, counselling and support to patients and families
• Discharge planning and coordination of referrals to community and support services
• Collaborate with multidisciplinary teams to optimise patient care and outcomes
• Participation in weekly MDT meetings in the rehabilitation unit

About you
