Job summary

Colac Area Health invites applications for a Social Support Facilitator (maternity leave relief). This temporary part-time role supports one-to-one and group programs to promote social connections and community engagement for carers and older adults.

Responsibilities include delivering diverse activities, maintaining client-centred care, and collaborating with a friendly team in a community-focused health service in Colac.

Qualifications

  • Minimum Cert III in Individual Support or equivalent.
  • First Aid Certificate Level 2.
  • Experience with older people and with participants with disabilities.
  • Current Driver’s Licence and Working with Children Check.
  • Excellent communication and time-management skills.
  • Ability to work independently and in a team.

Responsibilities

  • Deliver one-to-one and group social support programs for carer respite and community engagement.
  • Run engaging activities: exercise, outings, events, educational sessions, and volunteering.

About the role

SOCIAL SUPPORT FACILITATOR
Maternity Leave relief - Temporary Part Time - Up to 12 Hours per week until 14/03/2027

Colac Area Health is a values-based organisation providing a unique integrated health service comprising of over 500 employees, providing Acute Care, Aged Care, Community and Allied Health Services.

This role will provide a wonderful opportunity to expand our current innovative Social Support team that generate and conduct one to one and group programs for carer respite, social connections and community engagement within a friendly and safe environment.
Social Support engaging programs include exercise, outings, events, educational sessions, carer respite, volunteer visiting and are sportive of participants to maintain and improve their wellbeing and skills.

What’s required?

  • Minimum Cert III in Individual Support or its equivalent or working towards.
  • First Aid Certificate Level 2 or willingness to undertake.
  • Demonstrated rapport with older people and an understanding of their needs.
  • Experience of working with the aged as well as with participants with intellectual and/or physical disabilities.
  • Demonstrated understanding of the needs of aged people and the social experiences that will add value to their lives.
  • Knowledge of social support and other relevant services
  • Excellent personal and communication skills.
  • Excellent planning and organisational skills, including time management.
  • Knowledge of and experience with computer systems including MS Word, Publisher, Excel, Outlook and social media platforms.
  • Capacity to work as a sole worker and co-operatively within a team.
  • A positive, energised and encouraging outlook.
  • Experience of delivering a person-centred approach.
  • Experience with group work and managing group dynamics.
  • Current Driver’s Licence and Working with Children Check.
