Upwards Living is seeking compassionate Support Workers to provide high-quality, personalised support to participants under their care in alignment with individual support plans.

You will work closely with our Service Management team, maintaining independence, wellbeing and quality of life for participants. Availability for AM/PM and sleepover shifts across Monday to Sunday is required.

Formación

• English communication skills required.
• Experience as a SIL Support Worker or Assistant in Nursing (AIN).
• Certificate III in Individual Support/Aged Care, Certificate IV in Disability or Diploma in Community Services.
• Current NDIS Worker Screening Check.
• Current NSW Working with Children Check where applicable.
• Current First Aid and CPR.
• Valid driver’s licence.
• Valid Australian working rights.

Responsabilidades

• Assist with daily living activities and community participation.
• Monitor wellbeing and escalate concerns.
• Maintain accurate case notes and progress records.

What You’ll Be Doing

You will provide hands-on, person-centred support tailored to each participant’s individual needs and goals. Your responsibilities will include:

• Providing personalised care and support in accordance with individual support and care plans
• Supporting participants to build independence and work towards their individual goals
• Assisting participants with daily living activities and community participation where required
• Monitoring participants’ wellbeing and escalating concerns appropriately
• Maintaining accurate case notes, progress records and incident documentation
• Following organisational policies, WHS requirements and relevant NDIS and Aged Care standards
• Communicating effectively with participants, families, Service Managers and other members of the support team

What We’re Looking For

To be successful in the role, you must have:

• Ability to communicate in English
• Prior experience working as a SIL Support Worker or Assistant in Nursing (AIN)
• Certificate III in Individual Support/Aged Care, Certificate IV in Disability, Diploma of Community Services, or another relevant health services qualification
• Current NDIS Worker Screening Check
• Current NSW Working with Children Check, where applicable
• Current First Aid and CPR
• Valid driver’s licence
• Valid Australian working rights
• Strong verbal and written English communication skills
• A compassionate, respectful and person-centred approach
• Strong problem-solving skills and the ability to maintain professional boundaries

Experience supporting people with complex needs, including positive behaviour support, manual handling, epilepsy, diabetes, PEG feeding, catheter care or complex mental health needs, will be highly regarded.
