Job summary

New Life Shelter in New York is seeking a dedicated Case Manager to organize comprehensive service planning and assessment for residents. You will coordinate with housing, mental health, and healthcare providers to ensure integrated care and smooth transitions.

The role emphasizes crisis intervention, ongoing advocacy, and building individualized service plans with discharge planning goals. Onsite work schedule Sunday–Thursday, 9:00am–5:30pm, with comprehensive benefits.

Qualifications

• Bachelor's degree with practicum including activities with target population or major with 24 credits in social work, psychology, nursing, rehabilitation, education, or related fields; plus one year of case management or relevant human services experience.
• Associate's degree in health or human services field with three years of case management experience.
• High school diploma/GED with three years of direct service experience to individuals with mental disabilities or homeless individuals.

Responsibilities

• Participates in orientation of new residents and explains types of clinical services and programs.
• Documents assessment information including medical, psychological, and social factors contributing to residents' situations and strengths/needs.
• Develops resident service plans with an integrated healthcare approach, considering cultural and language needs.
• Provides one-to-one or group counseling to support behavior change and service needs.
• Assists residents with linking to community resources and coordinating with co-providers across housing, mental health, and healthcare services.
