Job summary

Launch Housing is seeking an Intensive Case Manager to deliver targeted outreach and intensive case work for clients facing homelessness. You will develop client-centered case plans, coordinate flexible engagement, and advocate for access to housing and services with partner agencies.

The role emphasizes recovery-oriented, harm-reduction approaches and collaboration within the Supportive Housing team. You will build on clients’ strengths, manage goals, maintain high-quality documentation, and

Qualifications

• Relevant tertiary qualification in human services or equivalent is preferred.
• Experience delivering services to clients with complex needs.
• Understanding housing and homelessness policy, context and systems.
• Ability to work collaboratively towards effective client solutions.
• Excellent verbal and written communication; strong organisational skills.
• Commitment to Launch Housing values and ending homelessness.

Responsibilities

• Assertive, persistent outreach to identify and engage rough sleepers to progress intensive case management.
• Develop case plans with clients and implement strategies to achieve goals.
• Advocate with other agencies to ensure access and service delivery.
• Contribute to innovative solutions to improve accommodation and housing outcomes.
• Ensure reporting requirements are completed to a high standard.
• Participate in team meetings and contribute to a positive, collaborative culture.
• Engage with internal and external stakeholders to broaden services.
