Job summary

Barossa Hills Fleurieu Local Health Network seeks a part-time Care Coordinator to support older people living in their own homes. You will coordinate flexible services, develop care plans with clients, and lead a team of community support workers to deliver timely, consumer-directed care.

The role offers collaboration with families and providers in a supportive environment, with opportunities to develop carecoordination and leadership skills within BHFLHN.

Qualifications

  • Experience coordinating client services in aged care or community settings with a person-centred or consumer-directed approach.
  • Ability to conduct home-based assessments, develop care plans, and prioritise referrals to meet client needs.
  • Ability to supervise, support and motivate community support workers, and manage rosters and changing client requirements.
  • Strong communication and interpersonal skills to build relationships with clients, families and diverse stakeholders.
  • Highly organised with the ability to manage competing priorities and maintain accurate records.

Responsibilities

  • Deliver high-quality, person-centred care coordination to help older people stay independent at home.
  • Undertake assessments, develop care plans and manage referrals for timely service delivery.
  • Lead and support community support workers, ensuring rostering, communication and quality outcomes.
  • Build relationships with clients, families and providers to ensure right care at the right time.
  • Maintain accurate documentation and contribute to continuous improvement and compliance.

  • Do you have experience coordinating client services in agedcareor a community setting, with a strong focus on person-centred or consumer-directed care?
  • Are you confident conducting home-based assessments, developingcareplans, and prioritising referrals to meet client needs effectively?
