Job summary

SA Health is seeking a part-time Community Services Coordinator in Coober Pedy to lead the Support at Home and CHSP programs. The role focuses on delivering age-friendly, person-centred services within the Coober Pedy community and allied health team.

The position runs temporarily through to 24 September 2027, with standard local conditions, pro-rata salary, and benefits. You will supervise staff and engage with partners to ensure timely service delivery.

Qualifications

• Experience in home and community care delivered in line with standards of practice.
• Coordinate person-centred services and manage program planning, implementation and evaluation.
• Supervisory experience within health or community services setting.
• Strong knowledge of needs of older people, disability, and carers.
• Familiarity with SA Health funding frameworks and primary health care principles.

Overview

SA Health's Eyre and Far North Local Health Network is seeking an experienced Community Services Coordinator to lead the delivery of the Support at Home and Commonwealth Home Support Program within the Coober Pedy Community and Allied Health team - on a part-time basis through to 24 September 2027.

What You'll Be Doing

• Coordinating and overseeing the day-to-day delivery of Support at Home (SAH) and Commonwealth Home Support Program (CHSP) services, ensuring targets and contractual obligations are consistently met
• Applying a holistic primary health care approach to develop and implement health promotion strategies that support older people to remain independent in their own homes
• Ensuring services are culturally appropriate and inclusive, with particular regard for the diverse communities across the Coober Pedy region
• Supervising staff and working collaboratively with government, non-government and community partners to coordinate participant services effectively

What You'll Bring

• Demonstrated experience in home and community care, delivered in line with relevant standards of practice
• Proven ability to coordinate person-centred services and manage program planning, implementation and evaluation
• Experience in a supervisory capacity within a health or community services setting
• Strong knowledge of the needs of older people, people living with disability, and their carers
• Familiarity with relevant legislation, funding frameworks, and the principles of primary health care - ideally within the South Australian public health context
• Confident use of Microsoft Office 365, including Teams, for service delivery and reporting
