Job summary

Barossa Hills Fleurieu Local Health Network is seeking two permanent part-time professionals for Mount Barker Hospital: a Registered Nurse Level 2 ANUM (RN/M2) and an Allied Health Professional (AHP2) in the Transitional Care Program.

These roles support consumers transitioning between care settings, delivering person-centred care within a multidisciplinary team, with development opportunities and a supportive regional workplace.

Qualifications

• Registration with AHPRA to practice as an RN or AHP, as applicable.
• Experience in case management, care coordination or rehabilitation services.
• Ability to collaborate with multidisciplinary teams to plan care.

Responsibilities

• Coordinate and deliver high-quality case management services that support consumers to achieve their goals.
• Develop, implement and monitor comprehensive care plans with consumers, carers and teams.
• Facilitate timely assessments and referrals for seamless transitions across care settings.
• Contribute to quality improvement, clinical governance and evidence-based practice.
• Provide leadership and guidance within a collaborative team environment.

Join our dedicated Transitional Care Program team at Mount Barker Hospital, with opportunities available for both a Registered Nurse Level 2 ANUM and an Allied Health Professional AHP2. These permanent part-time roles will directly support consumers as they transition between care settings, promoting independence, wellbeing and positive health outcomes. • Facilitate timely assessments, referrals and access to appropriate services, ensuring seamless transitions across the healthcare continuum.
• Contribute to quality improvement, clinical governance and evidence-based practice initiatives that enhance service delivery and consumer experience.
• Foster strong professional relationships and provide leadership, guidance and support to colleagues, students and less experienced staff within a collaborative team environment.
