Job summary

SA Health in Adelaide’s Southern Adelaide Local Health Network seeks a Senior Social Worker in the Community Geriatrics team. The role focuses on advanced assessments, care coordination and intervention for older adults living at home.

The position supports multidisciplinary teams, promotes person-centred care, and offers ongoing professional development within a growing community-based geriatric service.

Qualifications

• Degree in Social Work with eligibility for membership of the Australian Association of Social Workers.
• Experience as a social worker in a health setting.
• Experience delivering social work services to older people in community programs.
• Experience training students or junior staff.
• Commitment to person-centred care and continuous improvement.
• Strong communication, collaboration and stakeholder engagement skills.

Responsibilities

• Conduct comprehensive assessments using formal and informal tools.
• Develop, coordinate and review tailored care plans for complex needs.
• Facilitate access to community, health and support services.
• Build partnerships with clients, families and multidisciplinary teams.
• Provide expert clinical advice and advocacy to optimise outcomes.
• Lead outcome measurement and quality improvement activities.
• Mentor students and less experienced clinicians.
