Job summary

SA Health in Adelaide’s Southern Adelaide Local Health Network seeks a Senior Social Worker in the Community Geriatrics team. The role focuses on advanced assessments, care coordination and intervention for older adults living at home.

The position supports multidisciplinary teams, promotes person-centred care, and offers ongoing professional development within a growing community-based geriatric service.

Qualifications

• Degree in Social Work with eligibility for membership of the Australian Association of Social Workers.
• Experience as a social worker in a health setting.
• Experience delivering social work services to older people in community programs.
• Experience training students or junior staff.
• Commitment to person-centred care and continuous improvement.
• Strong communication, collaboration and stakeholder engagement skills.

About the role:

As a Senior Social Worker within the Community Geriatrics team, you will play a pivotal role in delivering person-centred care to older adults with complex medical, functional and psychosocial needs. Working within a case management model, you will provide advanced clinical assessment, care coordination and therapeutic intervention in a community setting.

Key responsibilities include:

• Conducting comprehensive assessments using a range of formal and informal assessment tools.
• Developing, coordinating and reviewing tailored care plans that address complex client needs.
• Facilitating access to appropriate community, health and support services.
• Building effective partnerships with clients, families, carers and multidisciplinary service providers.
• Providing expert clinical advice and advocacy to optimise client outcomes.
• Leading outcome measurement and evaluation activities to inform evidence-based practice and continuous service improvement.
• Supporting the development of students and less experienced clinicians through education, mentoring and clinical supervision.

About you:

• A degree or equivalent in Social Work, entitling membership of the Australian Association of Social Workers
• Demonstrated high level competency and extensive knowledge in Social Work assessment and therapeutic interventions to clients with complex needs with a broad range of Senior Social Worker clinical skills
• Experience as a social worker in a health setting
• Experience providing social work services to older people in a community programme
• Experience in the training of students and less experienced staff
• A commitment to person-centred care, clinical excellence and continuous improvement
• Strong communication, collaboration and stakeholder engagement skills
