Job summary

Royal Rehab Ryde is seeking a Senior Social Worker and Case Manager for the Brain Injury Unit (BIU). You will provide evidence-based clinical care, guide rehabilitation case management, and ensure smooth discharges back into the community.

You will work within a multidisciplinary team to advocate, problem-solve, and create meaningful change for clients and their families daily.

Qualifications

• Tertiary qualifications in Social Work; eligible for AASW membership; commitment to AASW Code of Ethics.
• Experience and clinical skills in assessment, counselling, advocacy, and discharge planning in health/rehabilitation.

Responsibilities

• Counselling to assist clients and carers with emotional impact of injury and disability.
• Case planning, liaison, advocacy, and referrals to maximise community service access.
• Liaise with multi-disciplinary team to optimise client care.
• Contribute to ongoing development of social work and case management services.
• Fulfills social work and case management duties; understands iCare schemes.

Key Responsibilities Include

• Counselling to assist clients and their family/carers to deal with the emotional impact of injury, illness and disability.
• To undertake case planning, liaison, advocacy and referral to maximise access to appropriate community services.
• Liaise with multi-disciplinary team colleagues to optimise client care.
• Contribute to the ongoing development of the social work and case management services.
• This role fulfills both the Social Work and Case Management aspects of service delivery. Sound understanding of the iCare (Workers Compensation and Lifetime Care Schemes) is essential.

About You..

We seek a self-motivated and friendly individual with:

Essential Criteria

• Tertiary qualifications in Social Work, eligibility for membership of the Australian Association of Social Workers and commitment to the AASW Code of Ethics.
• Experience and clinical skills - assessment, counselling, advocacy, referral to community services and discharge planning in a health/rehabilitation setting.
• Experience working in a health setting with adults with a physical or cognitive disability.
• Demonstrated competence in communication skills - written and verbal, negotiation, conflict resolution.
• Demonstrated commitment to participation in supervision and professional development.
• Computer skills and demonstrated competency.
• Sound understanding of the iCare (Workers Compensation and Lifetime Care Schemes).
