Job summary

Mid North Coast Local Health District in Port Macquarie invites applications for a Social Worker (Level 3) in the Transition Aged Care Program. Qualifications

• Eligible for membership of the Australian Association of Social Workers.
• Experience in aged care and post-discharge care is desirable.
• Demonstrated ability to lead care coordination within a multidisciplinary team.
