Job summary

Northern Sydney Local Health District seeks a passionate Social Worker to join the Community Aged Care team part-time. This role offers 24 hours per week in a hospital-based setting, delivering person-centred care and building community connections to improve outcomes for older adults.

Based at Royal North Shore Hospital, you will work within a multidisciplinary team under NSW Health policies, applying your social work expertise to assessments, care planning and advocacy for clients and families.

Qualifications

• Social Work degree and eligibility for the Australian Association of Social Workers (AASW).
• Ability to demonstrate a practical application of the AASW Code of Ethics.
• Understanding of ageing-related issues and care needs.

Responsibilities

• Provide high-quality Social Work service to consumers, carers and families.
• Support age-friendly community connections and person-centred care.
• Perform assessment, counselling, crisis management, case management, advocacy and referrals.
