Job summary

Western Sydney Local Health District – Westmead NSW is seeking a Social Worker Level 3 to lead complex discharge planning and community reintegration for older adults in an acute mental health setting.

You will join the Older Persons Mental Health Unit at Westmead Hospital, working with a multidisciplinary team to deliver person-centred, recovery-focused care while supporting carers and families.

Qualifications

• Tertiary qualifications in Social Work.
• Registration: Eligibility for Australian Association of Social Work (AASW) membership.
• Substantial experience and skills in Social Work assessment, planning and evaluating interventions for older consumers with complex mental health conditions.
• Ability to work independently and as part of a multidisciplinary team.
• Demonstrated knowledge of NSW Mental Health Act 2007 and related governance.

Responsibilities

• Provide advanced clinical social work assessment and intervention for older people with complex mental health and psychosocial needs.
• Lead and coordinate discharge planning, community reintegration and residential aged care placement.
• Apply advanced clinical judgement and specialist knowledge to support safe, person-centred care.
• Work across multidisciplinary teams and service networks, including reporting and supervision responsibilities.
