Job summary

Queensland Government's Social Work Department is seeking a skilled Social Worker to provide high-level assessment, case management, therapy and interventions across inpatient, outpatient and community settings.

You will collaborate with multidisciplinary teams, lead quality and service improvement initiatives, respond to complex needs, monitor outcomes and contribute to research, education and training while building strong relationships with patients, families and stakeholders.

Qualifications

  • Hold a recognised Social Work degree.
  • Eligible for AASW membership with ongoing professional development.
  • Use your full scope of practice to provide complex psychosocial assessment, advocacy and interventions across the continuum of care.

Our Social Work Department services hospital and community settings in Gladstone, Biloela, Emerald, the Capricorn Coast and Rockhampton, making a meaningful difference to people and families across Central Queensland.

About the role:

Provide high-level Social Work assessment, case management, therapy and interventions across inpatient, outpatient and community settings.

Collaborate with multidisciplinary teams and internal and external stakeholders to achieve the best outcomes for patients and clients.

Take responsibility for specific clinical portfolios and contribute to improving procedures, protocols, services and evidence-based practice.

Respond to complex and emerging needs, monitor clinical outcomes and help improve patient flow, access and quality of care, while contributing to a positive workplace culture.

About you:

Hold a recognised Social Work degree, be eligible for AASW membership and maintain ongoing professional development.

Use your full scope of practice to provide complex psychosocial assessment, advocacy and interventions across the continuum of care.

Lead quality and service improvement initiatives, from identifying opportunities for change through to implementation and evaluation.

Build strong relationships with patients, families, multidisciplinary teams and stakeholders, while contributing to research, education and training.

What we offer:
  • Total remuneration value of up to $157,359 p.a. is comprised of:
  • An hourly rate between $64.64 - $69.55 p.h.
  • 12.75% employer superannuation contribution
  • 17.5% leave loading
