Job summary

NSW Health’s Hospital in the Home service, based at Mona Vale Hospital, is seeking senior registered nurses to assess and treat patients in their homes or at one of our four clinics.

You will apply exceptional assessment and decision-making skills, coordinating with a multidisciplinary team to ensure safe, high-quality care. Opportunities may arise at Hornsby Hospital, RNSH or Northern Beaches Hospital.

Qualifications

• Registered Nurse – Division 1 with Australian Health Practitioner Regulation Agency (AHPRA) and relevant nursing experience.
• Demonstrated ability to communicate effectively with patients, carers and a diverse range of health and non-health care providers.
• Demonstrated ability to work well within a multidisciplinary team, with an emphasis on service delivery, patient safety, quality of care and team cohesion.

Responsibilities

• Assess, plan, implement and evaluate community nursing care in the patient home or clinic.
• Collaborate with individuals and the health care team to achieve quality patient outcomes.
• Engage respectfully with Aboriginal and/or Torres Strait Islander communities and promote inclusive care.
