Responsibilities

• Provide comprehensive, coordinated and person-centred nursing care to individuals and families within their homes and community settings.
• Deliver client-goal focused service to empower health outcomes.
• Provide chronic disease management, wound and catheter care, palliative care, health promotion and carer support.
• Collaborate with consumers, carers, GPs and multidisciplinary teams to ensure continuity of care.
