Job summary

Southern Cross Care WA is seeking a Senior Physiotherapist to join our Home Care team across Joondalup, Fremantle and Willetton. You will deliver high-quality, evidence-based physiotherapy, focusing on assessment, clinical reasoning and goal-driven interventions to support independence.

You must be AHPRA registered with 3–5 years’ experience, work well independently and in a team, and be committed to person-centred care.

Qualifications

• Current AHPRA registration as a Physiotherapist.
• 3–5+ years’ post-graduate physiotherapy experience.
• Experience in acute/chronic pain management, functional reablement and goal-based rehabilitation.
• Ability to work independently and within a multidisciplinary team.
• Excellent communication and documentation skills.

Responsibilities

• Deliver high-quality evidence based and outcome measured physiotherapy services to our clients including;
• Acute and Chronic pain management
• Functional based reablement
• Falls prevention/Recovery programs
• Chronic Disease and Multimorbidity Management
• Prescription of mobility aids
• Communicating effectively with the wider multi-disciplinary team to enhance the wellbeing of clients
• Contributing to quality improvements
