Job summary

BlueCare is seeking a Senior Personal Carer to deliver client‑centred care across Stanthorpe and surrounding areas. You will conduct intake, assess needs, and develop personalised care plans aligned with each client’s goals.

In this role you will perform home visits, assist with daily living activities, and coordinate transport and shopping support. You’ll use CRM/CMS to keep records current and participate in ongoing care planning with the team.

Qualifications

• Current Australian Driver's Licence (Green Provisional minimum) - Essential.
• Experience with technology incl. smartphones, tablets, laptops etc.
• Confidence to satisfy the probity checks required by legislation or policies.

Responsibilities

• Conduct initial client intake and screening assessments and develop care plans.
• Provide client-centred care including personal self-care, cleaning, social support, respite, transport and shopping.
• Maintain client information in CRM & CMS with up-to-date case notes.
• Engage in ongoing care discussions, planning, reviews and outcome evaluations with the care team.
• Travel to client homes for scheduled visits and assist with daily activities.
