BlueCare is hiring a Senior Personal Carer in Stanthorpe and surrounding areas. The role involves client intake, care plan development, and delivering in-home support across personal care, housekeeping, social activities, and transport.

You will manage client data in CRM/CMS and collaborate with the care team to review goals and outcomes. The position requires a Certificate III Aged Care or equivalent, a valid Australian driver’s licence, and a vehicle for work.

Qualifikationen

• Certificate III Aged Care, Individual Support or an equivalent qualification – Essential
• Current Australian Driver's Licence (Green Provisional minimum) – Essential
• Roadworthy & insured (Third Party Property minimum) vehicle to use for work – Essential
• Experience with technology incl. smartphones, tablets, laptops etc
• Confidence to satisfy the relevant probity checks required by legislation or policies

Aufgaben

• Conduct initial client intake and screening assessments and develop care plans with a client-centered approach
• Develop and review individualized care plans for personal self-care, housekeeping, social support, respite, transport and shopping assistance
• Manage client information in CRM & CMS ensuring up-to-date details and case notes
• Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals with the care team
• Spend time on the road completing scheduled visits and assisting clients in their homes
