Job summary

BlueCare is seeking a Senior Personal Carer to conduct comprehensive client intake, assessments and to develop personalised care plans. The role involves in-home support across personal care, house cleaning, transport and social activities, with documentation in CRM and CMS.

You will participate in planning, reviews and ongoing care discussions with the team while travelling to clients' homes across Stanthorpe and surrounds.

Qualifications

• Essential qualification: Certificate III Aged Care or equivalent.

Responsibilities

• Conduct client intake and screening assessments.
• Develop and review individualised care plans.
• Manage client information in CRM & CMS and keep case notes up to date.
• Provide in-home care including personal self-care, house cleaning, transport, shopping and social support.
• Engage in ongoing care discussions and reviews with the care team.
• Travel to scheduled visits and work on the road with clients.
