BlueCare, part of UnitingCare Queensland, is hiring a Senior Personal Carer in Stanthorpe to deliver high-quality, client-centred care. You will conduct initial intake, assess needs, and develop individualized care plans, including self-care, home support, transport, and social activities.

You will travel to client homes, maintain accurate records in CRM/CMS, and collaborate with the care team to review goals.

Qualifications

• Certificate III Aged Care or equivalent required.
• Current Australian Driver's Licence (Green Provisional minimum) required.
• Roadworthy and insured vehicle for work use.
• Experience with technology incl. smartphones, tablets, laptops.
• Ability to pass probity checks under legislation or policies.

Responsibilities

• Conduct initial intake and develop care plans for personal self-care and support needs.
• Manage client information in CRM & CMS with up-to-date notes.
• Engage in ongoing care planning, assessments, and reviews with the team.
• Travel to clients' homes for visits and daily activities support.
• Provide care according to each individual Care Plan.
• Support community-based recreational activities and transportation needs.
• Assist with shopping and appointments as required.

About the role:

As a Senior Personal Carer you'll conduct initial client intake and screening assessments and developing and reviewing individualised care plans with a client-centered approach while providing quality care based on clients' needs.

• Conduct thorough initial screening assessments, developing care plans for personal self-care, general house cleaning, social support, respite, transport and shopping assistance.
• Management of client information in CRM & CMS ensuring all details and case notes are kept up to date
• Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team and as required.
• Spending your time on the road completing scheduled visits, interacting one-on-one with clients within their homes supporting with daily activities
• Providing care in accordance with their individualised Care Plan
• Supporting with community based recreational and social activities
• Transportation and assistance with shopping and appointments.

Role requirements:
