BlueCare Australia is hiring a Senior Personal Carer to support clients in Logan Reserve, Mt Tamborine and surrounding areas. The role involves initial intake, assessments, and developing individualised care plans with a client-centred approach, delivering personal care, house cleaning, social support, transport and shopping assistance.

Responsibilities include updating CRM/CMS, ongoing care discussions, and home visits.

Formación

• Certificate III Aged Care, Individual Support or equivalent – Essential.
• Current Australian Driver's Licence – Essential.
• Roadworthy & insured (Third Party Property minimum) vehicle for work – Essential.
• Experience with technology incl. smartphones, tablets, laptops, etc.
• Confidence to satisfy probity checks required by legislation or policies.

Responsabilidades

• Conduct initial client intake and screening assessments and develop care plans.
• Provide care on a client-centred basis and support with personal self-care, general cleaning, social support, respite, transport and shopping.
• Maintain and update client information in CRM & CMS; participate in care planning and reviews.
• Spend time on the road with scheduled visits, providing one-on-one support in clients' homes.
• Assist with community activities and transportation as needed.

Remuneration: $37.77. p/hr + Super + Not for Profit Salary Packaging (Increases your take home pay) + Allowances & Travel Reimbursement (KM's) + Employee discounts

About the role:

As a Senior Personal Carer, you'll conduct initial client intake and screening assessments and developing and reviewing individualised care plans with a client-centered approach while providing quality care based on clients' needs.

Conduct thorough initial screening assessments, developing care plans for personal self-care, general house cleaning, social support, respite, transport and shopping assistance.

Management of client information in CRM & CMS ensuring all details and case notes are

kept up to date

Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals

and outcomes in consultation with the care team and as required.

Spending your time on the road completing scheduled visits, interacting one-on-one with clients within their homes supporting with daily activities

Providing care in accordance with their individualised Care Plan

Supporting with community based recreational and social activities

Transportation and assistance with shopping and appointments.

Role requirements:

Certificate III Aged Care, Individual Support or an equivalent qualification -Essential

Current Australian Driver's Licence (Green Provisional minimum) -Essential

Roadworthy & insured (Third Party Property minimum) vehicle to use for work-Essential

Experience with technology incl. smartphones, tablets, laptops etc

Confidence to satisfy the relevant probity checks required by legislation or policies
