Job summary

Stream Healthcare in Australia is seeking a Senior Clinician – Intake & Access Coordinator to lead the client access journey, conduct screening and triage, and connect clients with appropriate support. You will provide brief interventions and psychoeducation, and manage risk assessments with a trauma-informed, person-centred approach.

The role requires a tertiary health-related qualification and at least 5 years of clinical mental health experience, with strong communication and collaboration

Qualifications

• Minimum 5 years’ clinical experience within mental health or a related field.
• Experience in intake, access, assessment and clinical triage.
• Ability to determine client needs, urgency, eligibility and service pathways.
• Experience undertaking risk assessments and developing safety plans.
• Excellent communication and relationship-building skills.
• Compassionate, collaborative approach with trauma-informed practice.

Responsibilities

• Managing the client intake and access process from initial contact through referral or service engagement.
• Conducting screening, assessment and clinical triage to determine needs and service suitability.
• Assessing urgency, eligibility, therapeutic goals and support requirements.
• Providing brief interventions and psychoeducation where appropriate.
• Identifying and responding to complex clinical risks.
• Completing risk assessments and developing safety/response plans.
• Ensuring timely connection to the most appropriate program or service.
• Maintaining accurate clinical documentation and records.
• Collaborating with internal teams and external services to support outcomes.
