Job summary

BallyCara is seeking a Senior Clinical Care Partner to lead in community aged care and the Support at Home program. You will bring clinical leadership, governance, and person-centred care to clients in their homes.

You will report to the Integrated Community Manager, guiding a team of Care Partners, nurses and allied health professionals to deliver safe, high-quality services. This is an on-site role based in Brisbane, with opportunities to develop care practice and mentor others.

Qualifications

• Current AHPRA registration as a Registered Nurse.
• Experience in community aged care or Support at Home services.
• Strong leadership and coaching capabilities.

Responsibilities

• Provide clinical leadership and governance across Support at Home.
• Guide complex assessments, care planning and service coordination.
• Coach and mentor Care Partners to build capability.
• Ensure client-centred, safe care in home settings.
