Bolton Clarke Group is seeking a Clinical Care Partner to provide person‑centred, safe and effective care under the Support at Home program. You will deliver clinical oversight, coordinate complex care, and support participants to navigate services with a focus on quality and regulatory compliance.

Qualifications

• Current AHPRA Registration.
• Relevant care management experience in home/community care.
• Knowledge of community-based care and support services.
• Ability to manage a budget effectively.
• Demonstrated networking and liaison skills with providers and users.
• Excellent communication and interpersonal skills.
• Strong assessment and problem-solving abilities in complex care situations.

Responsabilités

• Develop individualised care plans with participants, goals and preferences.
• Coordinate and evaluate clinical services such as wound care and chronic disease management.
• Provide clinical oversight and ensure safety, quality and regulatory compliance.
• Maintain accurate clinical documentation and records.
• Support participants and carers in understanding services and decisions.
• Monitor budgets and adapt resource allocation as needs change.
• Escalate risks and incidents appropriately.
