Job summary

BallyCara is seeking an experienced Senior Clinical Care Partner to lead the Support at Home program in the Brisbane area. You will guide a multidisciplinary team, ensure safe, person-centred care, and uphold clinical governance while developing others.

The role is primarily leadership-focused with occasional direct client support to ensure continuity of care. You will report to Integrated Community Manager and collaborate across teams to improve outcomes for older Australians.

Qualifications

• Registered Nurse with AHPRA registration and current licensure.
• Demonstrated experience in community aged care, home care or Support at Home.
• Strong understanding of the Support at Home Program, including care management and funding arrangements.
• Knowledge of Strengthened Aged Care Quality Standards and relevant legislation.
• Experience leading, coaching and mentoring teams in clinical settings.
• Ability to balance clinical needs with client choice and service sustainability.

Responsibilities

• Provide clinical leadership, oversight, governance and team development across Support at Home.
• Support complex assessments, care planning, referrals and decisions.
• Identify clinical risks and promote safe evidence-based practice.
• Coach, mentor and support Care Partners and colleagues.
• Champion clinical governance and quality documentation.
• Build trusted relationships with clients, families and multidisciplinary teams.
